Provider Pledge

Family Equality requires all providers who wish to be listed in the LGBTQ+ Provider Database to carefully review the following pledge and have it signed by the leader of your organization, practice, or business as well as complete our Provider Intake form.

To be listed on Family Equality’s LGBTQ+ Provider Database:

My practice or business affirms that we have a non-discrimination policy that explicitly prohibits discriminating on the basis of sexual orientation, gender identity, and gender expression for staff and clients.

Additionally, Family Equality strongly encourages providers to abide by the following best practices to ensure LGBTQ+ prospective parents and families are welcomed, affirmed, and supported:

  1. Provide training and resources to all your staff on how to best welcome, affirm, and support LGBTQ+ families.
  2. Provide resources and services that address the specific needs of LGBTQ+ parents and their families.
  3. Have LGBTQ+ inclusive practices including but not limited to:
    • Inclusive forms that honor all LGBTQ+ parents such as asking for parents or caregivers names instead of lines for mom and dad; include intake questions noting sex-assigned-at-birth but also gender identity; include opportunities for clients to share their pronouns and preferred names; etc.
    • Inclusive facilities that include all gender restrooms as well as accessible changing tables and lactation spaces open to all genders, where possible. 
    • Privacy practices that ensure the sexual orientation and gender identity of all your clients is secure and protected and not disclosed without the client’s express permission.
  4. Publicly support LGBTQ+ families on your website, marketing, and/or your social media including but not limited to celebrations of Pride month, uplifting LGBTQ+ affirming stories, and advocating for LGBTQ+ inclusion in your industry and/or service area.

Lastly, to be listed in Family Equality’s LGBTQ+ Provider Database you understand and agree to the following:

  • Family Equality’s listing your organization, practice, or business herein is solely for educational purposes and does not imply endorsement by Family Equality or our partners, nor discrimination against similar organizations, practices, or businesses not mentioned.
  • Family Equality has the sole discretion to determine if your organization, practice, or business is listed on the LGBTQ+ Provider Database. Additionally, if Family Equality receives credible information of anti-LGBTQ+ practices by your organization, practice, or business, Family Equality has the sole and exclusive right to remove your listing from the Database. (Note, if we receive a complaint we will bring it to your attention and give you an opportunity to explain and/or correct the issue prior to removing you from the database.)
